Job Summary
The Case Manager will be responsible for providing comprehensive case management services to residents of Empowerment Dream Center. This includes assessing needs, developing individualized service plans, connecting residents to community resources, and supporting their progress towards self-sufficiency. The ideal candidate will have strong interpersonal skills, a background in social services, and a commitment to empowering vulnerable populations.
Key Responsibilities

Client Assessment and Planning

Conduct thorough assessments of residents' needs, strengths, and barriers.

Develop and implement individualized service plans in collaboration with residents.

Set realistic and achievable goals, and monitor progress regularly.

Resource Coordination
-
Connect residents with appropriate community resources and services, including housing, employment, healthcare, and education.

Advocate for residents with service providers and agencies to ensure access to necessary resources.

Maintain up-to-date knowledge of community resources and referral processes.

Support and Advocacy
-
Provide ongoing support and counseling to residents, addressing challenges and barriers to success.

Advocate for residents' rights and needs within the shelter and in the broader community.

Facilitate support groups and educational workshops on relevant topics.

Job and Home Search Assistance

Aid residents in searching for and securing stable employment opportunities.

Assist residents in locating and applying for suitable housing options.

Provide guidance on resume building, job applications, interview preparation, and housing applications.

Develop relationships with local employers and landlords to facilitate job and housing placements.

Documentation and Reporting

Maintain accurate and confidential case files, including assessments, service plans, progress notes, and referrals.

Prepare and submit required reports and documentation in a timely manner.

Utilize case management software to track and report on residents' progress and outcomes.

Collaboration and Teamwork

Work closely with shelter staff, volunteers, and other service providers to ensure coordinated and comprehensive care.

Participate in team meetings, case conferences, and training sessions.

Contribute to the development and improvement of case management policies and procedures.

Qualifications

Bachelor's degree in Social Work, Psychology, Human Services, or a related field required; Master's degree preferred.

Minimum of 2 years of experience in case management, preferably in a shelter or social service setting.

Strong understanding of issues related to homelessness, trauma, mental health, and substance abuse.

Excellent communication, organizational, and problem-solving skills.

Ability to work independently and as part of a multidisciplinary team.

Proficiency in case management software and Microsoft Office Suite.

Ability to work flexible hours, including evenings, weekends, and holidays as needed.

Ability to speak Spanish.
